Applying to the Herbal Apprenticeship Program

Applications may be submitted either online or by mail, and are evaluated in the order received until the class is filled. There is a $35 application fee.

Applying Online

To apply online, please do the following:

1. Fill out and submit the online application form.
2. Pay the application fee. When checking out, please put the applicant's full name into the Comments field at the botttom of the Secure Payment window and make sure that it matches the name given on the application form.

Applying by Mail

If you wish to apply by mail, please do the following:

1. Print out and fill out the hardcopy application form.
2. Enclose your payment, and mail it to the following address:
